Middlesex Health in Cromwell, CT is seeking a RN Triage/Weekend Coordinator for Homecare, 16 hours with alternating weekends. You will help ensure patients receive essential services during weekends and holidays and process referrals from all sources.

This role requires a CT RN license, strong computer literacy, and the ability to operate across multiple systems. Homecare experience is preferred, with on-call RN support as needed.

Department:Home Care Clinical Admin

As a Triage RN/Weekend Coordinator you will be responsible to insure that Home Care patients receive all available services on weekends and holidays in accordance with Middlesex Hospital objectives, standards and policies. Processes new referrals from all referrals sources.

Minimum Qualifications:

• A Registered Nurse, who is a graduate of an accredited School of Nursing, currently licensed to practice within the State of Connecticut.
• A non RN licensed clinician such as an Occupational or Physical Therapist may also fill this position with an RN administrator on call.
• All candidates must be computer literate and able to work is multiple systems.
• Must be able to multi-task and may be required to visit patients.
• Homecare experience
• Certification in specialty area preferred.
