Middlesex Health in Connecticut seeks an experienced RN to serve as Triage/Weekend Coordinator for Homecare. You will ensure weekend and holiday services are delivered in line with hospital objectives and policies, and you will process new referrals from all sources.

The role requires CT licensure as a Registered Nurse, comfort with multiple computer systems, and the ability to multi-task and visit patients as needed.

Qualifications

• Registered Nurse must be licensed to practice in CT.
• Non-RN licensed clinicians may fill this position with RN on call.
• Candidates must be computer literate and able to work in multiple systems.
• Must be able to multi-task and may be required to visit patients.
• Homecare experience preferred.
• Certification in specialty area preferred.

Department:Home Care Clinical Admin

As a Triage RN/Weekend Coordinator you will be responsible to insure that Home Care patients receive all available services on weekends and holidays in accordance with Middlesex Hospital objectives, standards and policies. Processes new referrals from all referrals sources.

Minimum Qualifications:

• A Registered Nurse, who is a graduate of an accredited School of Nursing, currently licensed to practice within the State of Connecticut.
• A non RN licensed clinician such as an Occupational or Physical Therapist may also fill this position with an RN administrator on call.
• All candidates must be computer literate and able to work is multiple systems.
• Must be able to multi-task and may be required to visit patients.
• Homecare experience
• Certification in specialty area preferred.
