Job summary

Middlesex Health is seeking an RN Triage/Weekend Coordinator for Homecare, 16 hours per week, including every other weekend, in Southington, CT. You will insure patients receive services on weekends and holidays, and manage referrals through multiple systems.

Responsibilities include triaging needs, coordinating home health services, and supporting the Home Care Clinical Admin department while ensuring compliance with hospital objectives and policies.

Qualifications

• RN licensed in Connecticut or eligible for licensure.
• Homecare experience preferred.
• Ability to work weekends/holidays and travel to patient sites if required.
• Computer literacy across multiple systems.

Responsibilities

• Triage and coordinate home health services on weekends and holidays.
• Process new referrals from all sources.
• Support Home Care Clinical Admin with coordination tasks.
• Ensure compliance with Middlesex Hospital policies and objectives.

Department:Home Care Clinical Admin

As a Triage RN/Weekend Coordinator you will be responsible to insure that Home Care patients receive all available services on weekends and holidays in accordance with Middlesex Hospital objectives, standards and policies. Processes new referrals from all referrals sources.

Minimum Qualifications:

• A Registered Nurse, who is a graduate of an accredited School of Nursing, currently licensed to practice within the State of Connecticut.
• A non RN licensed clinician such as an Occupational or Physical Therapist may also fill this position with an RN administrator on call.
• All candidates must be computer literate and able to work is multiple systems.
• Must be able to multi-task and may be required to visit patients.
• Homecare experience
• Certification in specialty area preferred.
