Middlesex Hospital invites applications for an RN Triage/Weekend Coordinator in Homecare. This role provides weekend coverage and coordinates services to ensure patients receive care on weekends and holidays in line with hospital policies.

The position requires CT-licensed registered nursing with homecare experience; non-RN clinicians may be considered with an on-call RN. Strong computer skills and ability to work across multiple systems are essential.

RN Triage/Weekend Coordinator Homecare 16 hours inc. every other weekend

Department:Home Care Clinical Admin

As a Triage RN/Weekend Coordinator you will be responsible to insure that Home Care patients receive all available services on weekends and holidays in accordance with Middlesex Hospital objectives, standards and policies. Processes new referrals from all referrals sources.

Minimum Qualifications:

• A Registered Nurse, who is a graduate of an accredited School of Nursing, currently licensed to practice within the State of Connecticut.
• A non RN licensed clinician such as an Occupational or Physical Therapist may also fill this position with an RN administrator on call.
• All candidates must be computer literate and able to work is multiple systems.
• Must be able to multi-task and may be required to visit patients.
• Homecare experience
• Certification in specialty area preferred.

• Compensation Range: $25.41 to $48.52
