Middlesex Health seeks an RN Triage/Weekend Coordinator for Homecare. This 16-hour role, including every-other weekend, ensures patients receive weekend services per hospital policies. You will process referrals and coordinate weekend coverage across departments.

CT-licensed RNs or qualified non-RN clinicians with on-call RN supervision are welcome. Strong computer and multitasking skills are essential.

Qualifications

• Must hold CT RN license or eligible to practice in CT.
• Non-RN clinicians may fill with RN on-call supervision.
• Computer literacy across multiple systems is required.

Responsibilities

• Ensure Home Care patients receive weekend/holiday services per policy.
• Process new referrals from all sources in a timely manner.
• Coordinate weekend coverage and on-call needs.
• May visit patients and support other clinicians as needed.

Department:Home Care Clinical Admin

As a Triage RN/Weekend Coordinator you will be responsible to insure that Home Care patients receive all available services on weekends and holidays in accordance with Middlesex Hospital objectives, standards and policies. Processes new referrals from all referrals sources.

Minimum Qualifications:

• A Registered Nurse, who is a graduate of an accredited School of Nursing, currently licensed to practice within the State of Connecticut.
• A non RN licensed clinician such as an Occupational or Physical Therapist may also fill this position with an RN administrator on call.
• All candidates must be computer literate and able to work is multiple systems.
• Must be able to multi-task and may be required to visit patients.
• Homecare experience
• Certification in specialty area preferred.
