Middlesex Hospital is seeking an RN Triage/Weekend Coordinator for Home Care. This 16-hour weekend role ensures patients receive essential services on Saturdays and Sundays, with on-call coordination for holidays.

You'll process referrals, work across multiple computer systems, and support Home Care Clinical Admin in line with hospital policies. CT-licensed RNs or qualified non-RN clinicians with on-call capability are welcome to apply.

Qualifications

• CT license to practice as an RN.
• Non RN clinician may fill with RN administrator on call.
• Able to work across multiple computer systems.
• Homecare experience is preferred.

Responsibilities

• Ensure Home Care patients receive services on weekends/holidays.
• Process new referrals from all sources.

Department:Home Care Clinical Admin

As a Triage RN/Weekend Coordinator you will be responsible to insure that Home Care patients receive all available services on weekends and holidays in accordance with Middlesex Hospital objectives, standards and policies. Processes new referrals from all referrals sources.

Minimum Qualifications:

• A Registered Nurse, who is a graduate of an accredited School of Nursing, currently licensed to practice within the State of Connecticut.
• A non RN licensed clinician such as an Occupational or Physical Therapist may also fill this position with an RN administrator on call.
• All candidates must be computer literate and able to work is multiple systems.
• Must be able to multi-task and may be required to visit patients.
• Homecare experience
• Certification in specialty area preferred.
