Job summary

HCA Healthcare is seeking an RN Coordinator in Virginia to manage patient transfers within the transfer center. Coordinate emergent and elective movements, match patient needs to facilities with bed capacity, and collaborate with referring facilities, physicians, and hospital staff.

The role requires an accredited nursing program graduate, a current RN license, and 3+ years in acute care. You will work in a fast-paced environment with cross-team collaboration and use multiple systems to ensure

Qualifications

• Graduate of an accredited nursing program required.
• Minimum of 3 years’ related clinical experience in an acute care setting (ED or Critical Care preferred).
• RN License in the states where the HCA facilities for that Division are located – multi-state license required if Transfer Center covers more than a single state (may be obtained upon hire).
• Knowledge of hospital and clinical operations and patient care practices.
• Strong problem solving, analytical and critical thinking skills.

Responsibilities

• Performs all transfer center functions in a timely, patient-centered manner using facility databases and protocols.
• Coordinates transfer of emergent, urgent and elective patients to final destinations across sources.
• Facilitates patient throughput with focus on quality and efficiency; provides follow-up.
• Demonstrates teamwork and professional communication with peers and staff.
• Ensures accurate data collection and maintenance in patient records and center workflow systems.
• Navigates technology platforms and documents patient information for beds and care plans.
• Applies critical thinking to assess situations and facilitate patient flow through care continuum.
• Communicates patient presentation clearly to accepting physicians and facilities.
