Job summary

Alliance Health Plan seeks a Supervisor II-Transition Support Specialist to oversee transitions of care, coordinate with care teams, and ensure compliant, efficient workflows. The role requires nursing or related licensed background with leadership experience in managed care settings.

The position is a full-time remote opportunity with occasional onsite meetings at the Alliance Office in Morrisville, North Carolina, to support coordination and strategic initiatives.

Qualifications

• Must hold an active clinical license as specified (NC).
• Two years of applicable experience with the target population.
• Experience with case management/discharge planning in relevant settings.

Responsibilities

• Provide supervision and staff development within the unit, coordinating with HR and management.
• Oversee transition-related data, referrals, and coordination of care across providers and plans.
• Coordinate with internal teams and external partners to ensure timely care transitions and accurate reporting.
• Monitor compliance with licensure, regulatory, and accreditation requirements.
• Promote a diverse, inclusive, and positive work environment and resolve staff conflicts.
• Lead meetings and reporting to leadership and stakeholders.
