Job summary

Alliance Health is seeking a Facility Based Care Managers Supervisor to lead a team delivering transitional care management across acute care, state centers, and justice system settings. The role is remote, with NC residency and travel to offices for meetings as needed.

Responsibilities include supervising staff and operations, ensuring compliance, driving quality improvement, and fostering collaboration with providers and stakeholders.

Qualifications

• RN license required with 5 years in population-served care management.
• Master’s in Human Services with 3 years post-grad experience in care management.
• Active clinical license in NC (LCSW/LMFT/LCAS/LCMH C/LPA or RN).
• Two years supervisory experience and CCM/CM certification preferred.

Responsibilities

• Supervise and develop staff with coaching and mentoring.
• Oversee unit operations and population-based program development.
• Provide outstanding customer service with providers and stakeholders.
• Utilize risk stratification and data to monitor care management performance.
• Ensure compliance with Alliance policies and procedures.
• Drive continuous quality improvement across the department.
• Travel between Alliance offices and to meet with members/providers as needed.
