Job summary

Alliance Health is seeking a Care Manager Extender to support Care Managers in delivering person-centered care coordination for members enrolled in the Tailored Plan. The role handles non-clinical tasks, outreach, and administrative duties ensuring timely and effective care management for individuals with complex health needs.

The position allows a primarily remote schedule, with occasional office visits for meetings as needed in North Carolina.

Qualifications

• Required: High school diploma or GED and 5+ years in healthcare, behavioral health, social services or care coordination.
• Or: Associate’s in human services and 3+ years in healthcare, behavioral health, social services or care coordination.

Responsibilities

• Conduct outreach calls to members for reminders, follow-ups, and engagement.
• Verify and update demographic information with member and online review; confirm guardianship status.
• Educate members about Tailored Care Management, Care Coordination, Care Teams.
• Assign members to appropriate Care Management/Care Coordination staff based on criteria to ensure timely access.
• Protect member confidentiality and comply with HIPAA and privacy standards.
