Job summary

AmeriHealth Caritas in Florence, SC is seeking a Care Manager II to coordinate care for Members with complex health needs. You will assess, plan, implement, and evaluate care, serving as the primary liaison for the care team and community partners.

This full-time role requires an accompanying RN license in South Carolina and a BSN, with at least 3 years in case management or related fields. Remote options and hybrid schedules are available, with competitive pay and benefits.

Qualifications

• BSN required and SC RN license
• Minimum 3 years in health care or related field
• Case management experience preferred, Foster Care experience preferred

Responsibilities

• Assess and coordinate care during transitions of care.
• Review medication lists and educate on pharmacy needs.
• Evaluate and update care plans based on Member progress.
• Establish contact points with community and health teams.
• Document Member interactions in care management platforms.
• Monitor utilization and coordinate services with payer sources.
• Develop knowledge of care management programs and regulatory requirements.
• Identify cases for care management rounds and follow up with providers.
• Support a positive workplace and collaborate with diverse populations.
• Conduct face-to-face visits for education and assessment as needed.
