Job summary

Alliance Health is seeking a Supervisor II Facility-Based Care Managers for a full-time remote role based in North Carolina (Morrisville area). The team provides transitional care management for members in acute, state-operated facilities, and justice system settings.

The candidate must reside in NC and travel to offices for meetings as needed. The position requires either a registered nurse license with 5 years of experience in relevant settings or a Master’s in human services with 3 years

Qualifications

• RN licensed with at least 5 years of experience in care management or discharge planning.
• Minimum of a Master’s degree in human services with 3 years post graduate experience in relevant settings.

Responsibilities

• Supervise and develop staff in facility-based care management teams.
• Oversee unit operations and ensure alignment with system strategy and values.
• Utilize data to monitor process improvement and ensure quality outcomes.
• Ensure compliance with policies, procedures, licensure, and regulatory requirements.
• Lead continuous quality improvement initiatives and cross-team collaboration.
• Travel to Alliance offices and meet with members, providers, and stakeholders as required.
