Job summary

Molina Healthcare is seeking a Registered Nurse to support the IRIS Self-Directed Personal Care program in Racine, Wisconsin. The RN will assess and oversee care plans and coordinate with IRIS Consultants to meet participant needs on a full-time day shift.

The role requires an Associate's degree in nursing, an active Wisconsin RN license, and a valid driver's license. Travel to member homes is required.

Qualifications

Associate's degree in nursing is required.
Active and unrestricted RN license in Wisconsin.
Valid and unrestricted driver's license.

Responsibilities

Provide personal care assessments and oversight using the Wisconsin Personal Care Screening Tool.
Document assessments as required by DHS policies.
Oversee a group of participants, developing individual plans of care.
Ensure physician orders for care are obtained and review/revise plans as needed.
Submit requests for Prior Authorization for personal care services.
Comply with all Department of Health Services policies and SDPC guidelines.
Provide personal care training to participants or care providers as requested.
Complete collateral contacts with IRIS Consultants and physicians to ensure care needs are met.
Complete other duties as assigned.
Drive 50% of the time for member home visits.
