Job summary

Molina Healthcare is seeking a Registered Nurse to support IRIS Self-Directed Personal Care across Wisconsin as a remote SDPC RN. You will oversee and educate participants, develop care plans, and ensure compliance with DHS policies while performing home visits as needed.

This role requires an active Wisconsin RN license, an Associate degree in nursing, and at least 2 years of nursing experience with older adults or disabled populations. Driving to member homes may be required.

Qualifications

• / Provides personal care assessments and oversight to My Cares Groups by administering Wisconsin Personal Care Screening Tool.
• / Documents assessment as required by policies and by completing oversight visits and calls.
• / Oversees My Cares Groups, develops care plans, ensures physician orders, and reviews plans.
• / Submits for Prior Authorization for personal care services.
• / Complies with policies and SDPC guidelines and regulations.
• / Provides personal care training and distributes educational materials.
• / Communicates with IRIS Consultants and physicians to ensure needs are met.

Responsibilities

• Provide oversight and guidance to IRIS SDPC participants.
• Develop individual plans of care and obtain physician orders.
• Document and follow up on care needs and assessments.
• Educate participants and care providers; provide training as needed.
• Coordinate with IRIS consultants and team to ensure program compliance.
