Job summary

Molina Healthcare is seeking a Registered Nurse for the IRIS Self-Directed Personal Care program in Wisconsin. This field-oriented role involves assessing and overseeing personal care for IRIS participants, with a focus on safety and compliance.

You will travel to member homes and collaborate with clinicians and care teams. The position requires a Wisconsin RN license, an Associate degree in nursing, and at least two years of nursing experience, including work with IRIS targets.

Minimum 2 years nursing experience
Experience with IRIS program is a plus

Responsibilities

Provide personal care assessments and oversight using the Wisconsin Personal Care Screening Tool.
Document assessments per DHHS policies.
Oversee participants, develop individual care plans and obtain physician orders.
Submit Prior Authorization requests for personal care services.
Comply with Department of Health Services policies and SDPC guidelines.
Provide personal care training to participants or care providers as requested.
Complete collateral contacts with IRIS Consultants and Long-Term Care Functional Screeners.
Drive 50% of the time during home visits.
Navigate various environments in members' homes.
