Job summary

Molina Healthcare in Marshfield, Wisconsin is seeking a registered nurse for the IRIS Self-Directed Personal Care program. This full-time role emphasizes hands-on personal care assessments, care planning, and collaboration with DHS policies while supporting individuals in their homes.

The RN will travel to member homes about half the time, perform assessments, document per guidelines, and coordinate physician orders and Prior Authorizations to ensure timely services.

Qualifications

• Associate's degree in nursing is required.
• At least 2 years of nursing experience, with 1 year serving IRIS targets.
• Current active RN license in Wisconsin.
• Valid unrestricted driver's license.
• Ability to drive roughly 50% of the time to member homes.

Responsibilities

• Provide personal care assessments using the Wisconsin Personal Care Screening Tool.
• Document assessments in line with tools and DHS policies.
• Oversee participants, develop care plans, and obtain physician orders.
• Submit Prior Authorization requests for personal care services.
• Comply with DHS policies and SDPC guidelines.
• Provide personal care training to participants or care providers as requested.
• Complete collateral contacts with IRIS Consultants and Long‑Term Care Functional Screeners.
• Perform other duties as assigned.
• Travel to member homes for visits approximately 50% of the time.
• Navigate various environments in members' homes.
