Qualifications

• Valid driver's license and reliable transportation for home visits.
• Experience with IRIS program or personal care services preferred.

Responsibilities

• Provide oversight and guidance to IRIS SDPC participants.
• Develop and review person-centered plans of care.
• Administer assessments and ensure proper documentation.
• Conduct training for participants and care providers as needed.
• Travel to member homes as required; collect collateral information.
