About This Role

The IRIS Self-Directed Personal Care (RN) position is ideal for a registered nurse seeking a unique opportunity to support individuals in living independently while maintaining a great work/life balance. This remote role involves partnering with participants in the Wisconsin IRIS program to provide oversight and guidance.

Highlights

• Pay: $26.41 - $51.49/hour
• Location: Oshkosh, WI
• Schedule: Full Time, Day shift

What You'll Do

• Provide personal care assessments and oversight by administering the Wisconsin Personal Care Screening Tool.
• Document assessments as required by individual tools and Department of Health Services policies.
• Oversee a group of participants, develop individual plans of care, and ensure physician orders for care are obtained.
• Submit requests for Prior Authorization for personal care services.
• Comply with all Department of Health Services policies and SDPC guidelines.
• Provide personal care training to participants or care providers as requested.
• Complete collateral contacts with IRIS Consultants and Long-Term Care Functional Screeners.
• Complete other duties as assigned.
• Drive 50% of the time during member home visits.
• Navigate various home environments, including stairs and potential exposure to pets.

Requirements

Education:

• Associate's degree in nursing required.

Experience:

• At least 2 years nursing experience, with 1 year serving the target groups of the IRIS program.

Licenses & Certifications:

• Active and unrestricted Registered Nurse (RN) license in Wisconsin.
• Valid and unrestricted driver's license.
