Job summary

Molina Healthcare is seeking a Registered Nurse licensed in Wisconsin for a remote IRIS Self-Directed Personal Care (SDPC) RN role. You will oversee care for individuals enrolled in the IRIS program, build relationships, and support person-centered plans of care from home with regular in-person visits.

Qualifications

• 2+ years nursing experience and 1+ year experience with IRIS program target groups (adults with disabilities or older adults) or equivalent
• Active Wisconsin RN license
• Associate's degree in nursing
• Valid driver's license with reliable transportation for travel as required
• Data entry and database skills
• Strong organizational and time-management skills
• Excellent written and verbal communication skills
• Proficiency with Microsoft Office and applicable software

Responsibilities

• Provide oversight and guidance to IRIS SDPC participants in their care plans
• Administer Wisconsin Personal Care Screening Tool and contribute to care plan development
• Coordinate assessments, visits, and documentation to ensure compliant care
• Submit prior authorizations for personal care services
• Educate participants and care providers on care plans and available resources
• Complete collateral contacts with IRIS consultants and physicians to meet care needs
• Manage multiple cases and adapt to changing program needs
• Occasional travel or home visits as part of the role
• Support home-based care customers while maintaining safety and quality standards
