Responsibilities

• Provide personal care assessments and oversight using the Wisconsin Personal Care Screening Tool.
• Document assessments per DHHS policies.
• Oversee participants, develop individual care plans and obtain physician orders.
• Submit Prior Authorization requests for personal care services.
• Comply with Department of Health Services policies and SDPC guidelines.
• Provide personal care training to participants or care providers as requested.
• Complete collateral contacts with IRIS Consultants and Long-Term Care Functional Screeners.
• Drive 50% of the time during home visits.
• Navigate various environments in members' homes.

What You'll Do

• Provide personal care assessments and oversight using the Wisconsin Personal Care Screening Tool.
• Document assessments as required by individual tools and Department of Health Services policies.
• Oversee a group of participants, developing individual plans of care and ensuring physician orders are obtained.

Requirements

Education:

• Associate's degree in nursing required.

Experience:

Licenses & Certifications:

• Active and unrestricted Registered Nurse (RN) license in Wisconsin.
• Valid and unrestricted driver's license and reliable transportation.
