Job summary

Qualifications

Responsibilities

• Provide personal care assessments and oversight using the Wisconsin Personal Care Screening Tool.
• Document assessments per tools and Department of Health Services policies.
• Oversee participants, develop individualized plans of care, obtain physician orders.
