Job summary

CareerNerds, LLC is seeking a proactive Risk Manager / Assistant Director of Nursing (ADON) for our premier long-term care facility in New York. You will lead clinical quality, oversee risk mitigation, and coordinate DOH reporting with the DON to maintain high standards of resident care.

The ideal candidate has an active RN license in NY, 3+ years in long-term care, and proven experience in DOH incident reporting and risk containment within a regulatory environment.

Qualifications

• Active, unrestricted RN license in the state of practice.
• Non-Negotiable: hands-on DOH accident/incident reporting experience.
• 3+ years RN experience in long-term care or similar setting.
• Experience as ADON, Risk Manager, QIC, or Clinical Supervisor in LT care.
• Deep understanding of DOH reporting mandates and state surveys.

Responsibilities

• Direct investigation, documentation, and mandatory reporting of all accidents, incidents, and DOH events.
• Partner with DON to oversee daily nursing operations and maintain high clinical quality.
• Conduct root-cause analyses and implement corrective action plans.
• Ensure compliance with DOH and other regulatory requirements; lead survey readiness.
• Educate and train staff on safety protocols, risk reduction, and incident documentation.
