Job summary

Clear Concepts, Inc. is seeking a proactive Risk Manager / Assistant Director of Nursing (ADON) to lead clinical quality and risk mitigation in our premier Long-Term Care facility in New York.

You will direct the investigation, documentation, and mandatory reporting of all accidents and reportable events to the Department of Health (DOH), and partner with the DON to oversee daily nursing operations while maintaining high clinical standards.

Qualifications

• Active RN license in NY and unrestricted practice.
• Hands-on experience submitting accident/incident reportables to DOH.
• 3+ years RN experience in long-term care (LTC).
• Experience as ADON, Risk Manager, or Clinical Supervisor in LTC.
• Solid knowledge of DOH reporting mandates and surveys.

Responsibilities

• Direct investigations and documentation of all facility accidents and reportables.
• Partner with DON to oversee daily nursing operations and ensure high clinical quality.
• Perform root-cause analyses and implement corrective action plans.
• Ensure compliance with DOH regulations and lead survey readiness.
• Educate staff on safety protocols and proper incident documentation.
