Job summary

Top Echelon Software LLC seeks a Risk Manager / Assistant Director of Nursing (ADON) for a premier long-term care facility in New York. The role leads clinical quality and risk mitigation, directing accident investigations and mandatory DOH reporting.

The candidate will collaborate with the DON, drive compliance, and mentor staff on safety protocols and documentation. RN licensure and DOH reporting experience are essential.

Qualifications

Active RN license in the state of practice.
Non-Negotiable: hands-on DOH reporting experience.
3+ years RN experience in long-term care or similar setting.
Experience as ADON, Risk Manager, QI Coordinator, or Clinical Supervisor in long-term care.
Deep understanding of DOH reporting mandates and state surveys.

Responsibilities

Direct investigation and reporting of all facility accidents, incidents, and reportable events to the DOH.
Partner with DON to oversee daily nursing operations and ensure high clinical quality.
Perform root-cause analyses and implement corrective action plans.
Ensure compliance with federal, state, and local DOH regulations and survey readiness.
Educate and train nursing staff on safety protocols and incident documentation.
