Job summary

Aspire Talent Solutions is seeking a proactive Risk Manager / Assistant Director of Nursing (ADON) for a premier long-term care facility in New York. The role leads clinical quality and risk mitigation, with direct experience submitting accident and incident reportables to the Department of Health (DOH).

You will partner with the DON to oversee nursing operations, maintain high clinical standards, and conduct root-cause analyses to drive immediate corrective actions.

Qualifications

• Active RN license in NY or applicable state.
• Hands-on DOH incident reportables submission experience is required.
• Minimum 3+ years of RN experience in long-term care or similar setting.
• Experience as ADON, Risk Manager, or Quality Improvement role preferred.
• Strong understanding of DOH reporting mandates and compliance requirements.

Responsibilities

• Direct investigation, documentation, and mandatory reporting of all accidents and incidents to DOH.
• Partner with Director of Nursing to oversee daily nursing operations and quality standards.
• Perform root-cause analyses on clinical incidents and implement corrective actions.
• Ensure compliance with federal, state, and local regulations and lead survey readiness.
• Educate and train staff on safety protocols and incident documentation.
