Job summary

Qualifications

• Licensed RN in Ohio.

Responsibilities

• Perform holistic assessments of patients' physical, psychosocial, and education needs.
• Develop and coordinate care plans with patients and families.
• Communicate plans using SBAR and participate in handoffs.
• Provide education to patients and families and ensure safe care delivery.
• Document nursing care accurately in the EMR and ensure continuity of care.
