Job summary

Qualifications

• RN license in Ohio is required.
• BSN within 5 years of hire is preferred for newer hires.
• BLS certification is required; ACLS within 6 months in step-down/ICU areas.
• Hospitals use SBAR and structured handoffs for care planning.

Responsibilities

• Perform holistic patient assessments across physical and psychosocial domains.
• Develop, coordinate, and revise multidisciplinary plans of care with patient/family.
• Educate patients and families throughout the care trajectory.
• Document care accurately in the EMR and communicate with the care team.
• Respond to emergencies with prompt, safety-conscious actions.
• Participate in discharge planning and resource referrals.
