Job summary

The Christ Hospital Health Network is seeking a registered nurse to provide comprehensive patient care within its Ohio facilities. This role emphasizes holistic assessment, collaborative care planning, and patient-family education in line with the hospital’s Professional Practice Model.

Responsibilities include SBAR communication, discharge planning, and rapid response to emergencies while maintaining safety and quality standards.

Qualifications

• RN must hold current Ohio nursing license.
• BLS certification is required prior to patient contact.
• ACLS certification required within 6 months for step-down and ICU areas.
• Certification in area of clinical specialty required within five years of eligibility.
• YEARS OF EXPERIENCE: None required.

Responsibilities

• Perform holistic patient assessments using The Christ Hospital Professional Practice Model.
• Coordinate and revise multidisciplinary plans of care with patient and family; set measurable goals.
• Communicate the plan of care using SBAR and handoff processes with the health team.
• Initiate discharge planning and make appropriate referrals.
• Provide ongoing, individualized patient and family education with the multidisciplinary team.
• Respond to emergencies with immediate, controlled action.
• Implement infection prevention measures and safety protocols.
