Job summary

Enhabit Home Health & Hospice is seeking a licensed professional (nurse or PT) to review patient information at start of care, recertification, resumption of care, or evaluation visit. The role requires current licensure and CPR certification, with at least one year of relevant experience.

The position collaborates with branch leadership to educate LPs and ensure documentation meets regulatory standards, while promoting high-quality home-based patient care and sustaining essential relationships

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Current valid clinical license as required by state rules.
• Minimum one year of experience.
• CPR certification is required.
• Valid state driver’s license and automobile liability insurance.
• Medicare home health or hospice experience preferred.
• Strong technology skills including MS Office.
• In-office attendance is essential (remote roles excluded).

Responsibilities

• Review and approve patient information submitted by the LP during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to educate and train LPs to ensure documentation meets regulatory standards.
• Provide one-on-one training when needed.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
