Qualifications

• Graduate of an accredited nursing program.
• BSN or enrollment in an accredited BSN Completion Program accepted.
• Current Registered Nurse License in the state of practice, with ACM certification preferred.

Responsabilités

• Provides case management, advocacy, and care coordination to hospital patients to ensure seamless transitions and reduce readmission risk.
• Conducts early patient assessment to identify discharge planning needs.
• Creates individualized patient-focused plans for continuing care needs.
• Makes referrals to post-hospital care providers using community resources.
• Collaborates with healthcare team to ensure timely discharge and complete patient education and instructions.
