Job summary

Beth Israel Lahey Health is seeking a Patient Flow, Admissions & Discharge RN to coordinate admissions and discharges on high-volume units. Under the ACNO supervision, you will conduct admission assessments, perform medication reconciliation, educate patients and families, and ensure safe, efficient transitions into the inpatient setting.

Qualifications

• Graduate of a state-approved and/or accredited RN program required.
• Current Registered Nurse (RN) License in the State of Massachusetts.
• Minimum of 1–2 years of clinical nursing experience.

Responsibilities

• Own admissions and discharges on designated units; ensure timely and thorough patient onboarding.
• Complete admission assessments and medication reconciliation; provide discharge instructions.
• Educate patients and families about medications, follow-up care, and transportation plans.
• Coordinate discharge planning with nursing supervisor, providers, and bedside nurses; promote safe transitions of care.
• Improve bed utilization and patient throughput by reducing delays in admissions and discharges.
