Job summary

Beth Israel Lahey Health is seeking an RN Case Manager to lead care transitions within the Triad Model of Care Transitions. You will partner with the interdisciplinary team to facilitate throughput, discharge planning, and safe patient progression through hospitalization.

The role requires MA RN licensure, 3–5 years in acute care, and certifications such as ACM/CCM/CMAC preferred, with BLS mandated. Strong communication and regulatory knowledge are essential.

Qualifications

• Education: MA RN licensure in Massachusetts.
• Preferred: Bachelor’s degree in nursing or healthcare field.
• Experience: 3–5 years in an acute care setting.
• Certifications: ACM, CCM, or CMAC preferred; BLS required.

Responsibilities

• Reviews admissions/bed placement within 24–48 hours and ongoing daily review to plan care progression and LOS.
• Collaborates with medical team to formulate treatment plan including care transitions and patient flow.
• Assesses admissions to identify barriers to discharge and resources needed for transitions.
• Coordinates with physician and care team to ensure tests and procedures are timely and appropriate.
• Communicates plan of care to patients, families, and multidisciplinary team; facilitates smooth transitions and follow-up.
