Job summary

Beth Israel Lahey Health is seeking an RN Case Manager to partner with the interdisciplinary care team and facilitate safe and timely discharge for hospitalized patients. You will drive throughput, clinical effectiveness, and fiscal responsibility while navigating health system services for effective transitions.

Responsibilities include developing the plan of care, coordinating the overall transition plan with social workers and departments, and ensuring effective communication with patients

Qualifications

• RN licensure in the state of Massachusetts
• Bachelor’s degree in nursing or another healthcare-related field is preferred
• 3–5 years in an acute care setting preferred

Responsibilities

• Develops plan of care and patient flow in collaboration with health care team.
• Coordinate transition planning and discharge processes.
• Identify barriers to length of stay and ensure timely care progression.
• Communicate plan of care and discharge needs to patient/family and team.
