Job summary

The Coordinator-RN PACE Intake at Careerwebsite drives enrollment and referral activity to promote the PACE program within the site area. You will educate referral sources, meet with physicians and discharge planners, and collaborate with the interdisciplinary team from referral to evaluation to ensure a smooth process for participants.

Education includes a Bachelor’s in Nursing with a California RN license and BLS certification; a driver’s license is required.

Qualifications

• Bachelor’s Degree in Nursing required.
• Minimum two years of nursing or case management experience.
• Valid California RN license; BLS certification; valid driver’s license.
• Proficient in English communication and basic computer skills (Word, Excel, Outlook, PowerPoint).

Responsibilities

• Drive enrollment and refer patients to promote PACE program growth.
