Job summary

Habitat Health, in California, is seeking an Enrollment RN to lead the clinical enrollment across the PACE program. You will partner with Outreach and IDT teams to conduct nursing, home safety, and eligibility assessments in participants' homes, ensuring compliant, patient-centered enrollment decisions.

This role requires an Associate's degree in Nursing, an active RN license, 1 year of geriatric experience, and strong communication.

Qualifications

• Associate's degree in Nursing required from an accredited program.
• Active, unrestricted RN license in the applicable state.
• Minimum of 1 year nursing experience with older adults or frail populations.
• Ability to conduct clinical and home safety assessments with sound judgment.
• Experience using electronic health records (EHRs).
• Ability to travel throughout the service area; valid driver's license.
• CPR/BLS certification or ability to obtain within 30 days of hire.

Responsibilities

• Partner with the Outreach Specialist and IDT to support enrollment experiences.
• Conduct nursing, home safety, and eligibility assessments in participants' homes.
• Complete NFLOC assessments and documentation per DHCS requirements.
• Present clinical findings at enrollment meetings with the IDT.
• Educate prospective participants and caregivers for informed decisions.
• Maintain accurate clinical documentation in the EHR and related systems.
• Stay updated on PACE eligibility and enrollment processes.
• Support outreach activities and community engagement as needed.
• Monitor enrollment activities and identify workflow improvements.
• Perform other related duties as assigned.
