Qualifications

• 2 years of nursing or case management experience required.
• 1 year minimum experience with frail and elderly population preferred.
• Knowledge of Medicare/Medicaid regulations a plus.

Responsibilities

• Meet with prospective enrollees to determine eligibility for the PACE program.
• Responsible for the Home Visit process including initial intake assessments.
• Coordinate services with State Health and Human Services Personnel.
• Participates in marketing activities to enhance participant referrals.
