Job summary

Loma Linda University Health is seeking a Coordinator-RN PACE Intake to drive enrollment and refer patients to promote growth of the PACE program within the site area. You will educate referral sources about the full spectrum of PACE services and meet with physicians, discharge planners, and post-acute leaders to generate referrals.

The role requires a Bachelor’s degree in Nursing and at least two years of nursing or case management experience; strong communication, organization, and teamwork

Qualifications

• Bachelor's Degree in Nursing is required.
• Minimum two years of nursing or case management experience required.

Responsibilities

• Drive enrollment and refer patients to promote program growth and market awareness.
• Educate referral sources about the full spectrum of PACE services including meetings with physicians and discharge planners.
• Develop and sustain referral relationships and meet enrollment goals.
• Collaborate with the Interdisciplinary Team from referral to evaluation.
• Participate in community education and events to raise program visibility.
