Job summary

Valir PACE is seeking a PRN Registered Nurse to support the PACE program as an RN Care Coordinator and on-call nurse. You will deliver comprehensive nursing care, assess participants, and coordinate services in homes and the PACE center.

Responsibilities include triage, medication management, and collaboration with the IDT to promote independence, safety, and quality of life for aging participants and enrollees.

Qualifications

• Current RN license in good standing.
• CPR certification is required.
• Valid Driver’s License for field-based work.
• Experience in geriatric, home health, or PACE preferred.

Responsibilities

• Provide skilled nursing care in PACE center and participant homes.
• Coordinate care plans with IDT and caregivers.
• Perform intake assessments and field-based services for PACE participants.
• Manage medication and monitor chronic conditions.
• Participate in after-hours triage and on-call responses.
• Ensure documentation and regulatory compliance.
