Job summary

Valir PACE is seeking a PRN Registered Nurse to support the PACE program as an RN Care Coordinator and on-call nurse. You will deliver comprehensive nursing care, assess participants, and coordinate services in homes and the PACE center.

Responsibilities include triage, medication management, and collaboration with the IDT to promote independence, safety, and quality of life for aging participants and enrollees.

Qualifications

This position delivers comprehensive nursing care, care coordination, participant assessments, field-based services, and intake evaluations for PACE participants and prospective enrollees. As a key member of the Interdisciplinary Team (IDT), the PRN RN collaborates with providers and team members to develop and implement individualized plans of care that promote participant independence, safety, and quality of life.The PRN RN provides skilled nursing services in the PACE center and participant homes while ensuring timely communication, documentation, and follow-up. Responsibilities may include participant triage, urgent clinical assessments, medication management, chronic disease monitoring, care transitions, after-hours telephone triage and on-call response, participant and caregiver education, and coordination of services to prevent avoidable hospitalizations and support successful aging in place.The RN ensures compliance with federal, state, and CMS PACE regulations while delivering participant-centered, high-quality nursing care throughout the participant lifecycle—from intake through ongoing enrollment.Education, Licenses, Certifications and Experience:Current Registered Nurse (RN) license in good standing (state-specific).Current CPR Certification.Valid Driver’s License (for field-based work).ACLS and UCAT certifications preferred (as applicable).Experience in geriatric, home health, or PACE preferred.Prior experience with in-home assessments and care coordination strongly preferred.Physical Requirements:Ability to travel regularly by vehicle.Lift up to 50 lbs (occasional).Walk up to 0.25 miles (frequent/occasional).Stand, sit, bend, squat, kneel, and reach as required.Push/pull up to 75 lbs of force (occasional).Full range of motion and balance for patient care and field work.
