Job summary

Fallon Health is seeking a Registered Nurse for Summit ElderCare, a PACE program, to deliver direct nursing care to participants across settings. The role requires strong collaboration with the IDT, care planning, and documentation per policy.

The nurse will advocate for patient needs, lead in clinical tasks, and ensure timely, safe care while adhering to program standards and MA regulations.

Qualifications

• Graduate of an accredited school of nursing.
• Current MA RN license required.
• CPR and Alzheimer’s certification, or willingness to obtain within 60 days.
• Reliable transportation to service area (30-mile radius).
• 2+ years direct care experience for adults/elderly; ambulatory or home care helpful.

Responsibilities

• Provide direct nursing care to PACE participants.
• Advocate for the participant’s best care and coordinate with the care team.
• Develop and implement individualized plans of care.
• Document all aspects of participant care and changes in health status.
• Participate in IDT meetings and site committees.
• Perform medication administration and nursing assessments as per policy.
