Job summary

Fallon Health is seeking a Registered Nurse to provide direct nursing care for PACE participants across settings, following nursing process and standards. You will collaborate with the interdisciplinary team to ensure high-quality, person-centered care and timely interventions for diverse participants.

As part of Summit ElderCare, a PACE program, you will support aging individuals to live safely at home while coordinating with Medicare and Medicaid programs.

Qualifications

• Graduate of an accredited school of nursing.
• Current Massachusetts RN license.
• CPR and Alzheimer’s certification, or willingness to be certified within 60 days.

Responsibilities

• Reviews and implements Provider’s orders.
• Conducts nursing assessments per policy guidelines including physical, psychosocial, behavioral, and MDS-HC.
• Delivers care to participants in any setting, including skilled services, based on individualized needs.
• Participates in the development and ongoing review of each participant’s care plan.
• Documents all aspects of participant care and health status changes in the medical record.
• Participates in the nursing on-call rotation and in the IDT process.
