Job summary

Fallon Health is seeking a Home Health Aide to provide in-home care and center-based support through Summit Eldercare (PACE). You will assist with daily living activities, participate in care planning, and help maintain a safe environment for participants in both home and center settings.

Under RN supervision, you will document patient status, support therapies, and promote infection control in line with CDC guidelines.

Qualifications

• Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is desirable.
• Certification as Home Health Aide or Nursing Assistant is preferred.
• Reliable transportation required.

Responsibilities

• Under RN supervision, assist with ADL needs for participants at the PACE center and in homes.
• Contribute to care plan development with Interdisciplinary Team.
• Collaborate to ensure appropriateness and continuity of care.
• Carry out non-skilled treatments including vital signs, transfers, toileting, bathing, dressing.
• Assist with programming and activities to motivate participation and evaluate effectiveness.
• Support registered therapists with ordered treatments and activities.
• Maintain a clean, safe environment at the PACE center and in homes.
• Assist with meal preparation, serving, and cleanup.
• Document accurately in EMR per policy.
