Qualifikationen

• Associate or bachelor of nursing degree required.
• Minimum of 1 year of RN experience required.
• Experience with obstetric care and care coordination preferred.

Aufgaben

• Coordinate care across prenatal, perinatal, and postpartum periods.
• Track appointments, referrals, and delivery planning to ensure continuity.
• Provide patient outreach, education and follow-up on care plans.
• Identify barriers to care and connect patients with resources.
• Perform nursing assessment and telephone triage with clinical judgment.
• Collaborate with OB providers, hospitals and community partners.
• Maintain timely documentation and support safety and quality initiatives.
• Provide occasional direct clinic nursing support as needed.
