Job summary

The position involves traveling to remote clinics in middle Georgia, patient education, care navigation, and collaboration across obstetrics, cardiology, pharmacy, and support services. Requires RN licensure and 5 years' relevant experience; DOH billing and reporting familiarity preferred.

Qualifications

• RN license in the state of practice; RNC-OB preferred.
• Typically 5 years of nursing experience in obstetric, maternal-fetal medicine, or cardiovascular nursing.
• Experience in care coordination or rural health outreach is a plus.

Responsibilities

• Ensure continuity of care across the cardio-obstetric care continuum from prenatal to postpartum.
• Develop relationships with patients and care teams to facilitate navigation and treatment.
• Identify and enroll obstetric patients with cardiovascular risk factors into the program.
• Complete nursing assessments focusing on obstetric and cardiac care; coordinate with multiple departments.
• Track diagnostics and report results to ensure timeliness and follow-through.
• Educate patients and families on cardio-obstetric conditions and disease management.
• Travel to remote clinic locations to support program delivery and outreach.
