Job summary

Centene Corp. is seeking a Care Manager (RN) to support OB programs remotely within Missouri. The role develops, assesses, and facilitates complex care management for pregnant members, with a Monday–Friday 8am–5pm schedule from home.

Requires RN license and a Bachelor’s degree in Nursing with 2–4 years of related experience. You will educate members and families, coordinate with providers, address social determinants, and ensure timely access to needed services while maintaining compliance.

Qualifications

• Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2–4 years of related experience.
• RN license in good standing per state requirements or compact licensure.

Responsibilities

• Assess member needs and barriers to accessing appropriate care.
• Develop and update care plans and service plans.
• Coordinate with providers, specialists, and community resources.
• Provide education to members and families about disease processes and care options.
• Monitor member status and progress toward goals; adjust plans as needed.
• Ensure compliant documentation and resource referrals.
