Qualifications

• RN licensure in the state or a compact licensure.
• Bachelor's degree in Nursing or equivalent required.
• 2–4 years of related experience in care management or nursing.

Responsibilities

• Evaluate member needs and barriers to accessing care.
• Develop ongoing care plans and collaborate with providers.
• Identify problems/barriers to care and provide interventions.
• Coordinate between members, families, and care teams.
• Monitor member status and care plan goals.
• Provide resource support for local services.
• Facilitate care management with providers for timely access.
• Perform telephonic, digital, or home outreach.
• Document and maintain member information for compliance.
• Educate members on disease processes and healthcare options.
