• Performs a nursing assessment to identify individualized needs and collaborates with the team to develop an interdisciplinary plan of care.
• Provides education, addresses psychosocial needs, advocates, and supports continuity of care.
• Coordinates timely, seamless, evidenced-based care across the care continuum to achieve optimal patient outcomes.
