Southcoast Health in New Bedford, MA is seeking an RN-Care Navigator to coordinate care across the continuum within the OBGYN service line. The role reports to Medical Director and supports patients with education, resources and follow-up to meet their care goals.

The position requires a BSN or equivalent, MA/RI RN licensure, BLS, 4 years of clinical experience including ambulatory work, and the ability to multi-task in a busy environment.

Qualifications

• Graduate of an accredited School of Nursing or equivalent is required.
• A Bachelor of Science (BSN) in Nursing or the equivalent education and experience is required.
• Four (4) years of clinical experience with at least 2 years in a related setting is required.
• Previous training as a care navigator, coordinator, or manager is highly desirable.
• Previous ambulatory/outpatient experience is preferred.
• Ability to multi-task, handle stressful situations in a fast-paced environment required.
• Knowledge and clinical expertise in community services available to patients and their families required.
• Current RN Licensure in the Commonwealth of Massachusetts and/or Rhode Island is required.
• Basic Life Support (BLS) certification is required.
• Oncology and/or GYN experience preferred.
• Obtain additional certification as indicated for assigned patient population within two years.
• Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.

Responsabilités

• Provide consistent care coordination across the continuum, assessing the physical, psychological and social needs of the patient.
• Serve as ongoing resource for patients throughout the care continuum.
• Organize and facilitate clinical visit needs.
• Assist patients and families by providing education, support and information regarding available resources for assigned patient population within the Care Center and Southcoast community.
• Provide educational materials and referrals to appropriate support groups and services.
• Support patient and families to ensure compliance with care.
• Create, monitor, and improve clinical quality indicators for program evaluation and any applicable regulatory or accreditation requirements.
• Facilitate prospective and/or retrospective case conferences, documentation of conference outcomes including treatment recommendations for each patient.
