Job summary

Compass Health Network is seeking a Community Health Worker in Columbia, MO. This full-time role supports primary care by connecting individuals to healthcare, social services, and community resources. Schedule is Monday–Friday, 8:00am–5:00pm, with duties spanning education, advocacy, and care coordination.

You will help patients navigate systems, support adherence, and promote wellness in underserved rural communities, aligning with our mission to improve lives and advance health equity.

Qualifications

• High School/GED required.
• Bachelor's degree preferred.
• Complete accredited Community Health Worker program (licensure/certification).

Responsibilities

• Serve as liaison between patients, providers, and community resources.
• Assist patients with navigating healthcare, insurance, and social service systems.
• Provide culturally appropriate education on health and wellness topics.
• Support medication adherence, follow-up care, and self-management goals.
• Help patients overcome barriers by arranging transportation, coordinating care, and advocating for needed services.
• Document interactions and participate actively in the primary care team.
• Build trust and promote health literacy within the community.
• Support patients during post-hospitalization or after missed appointments.
• Implement and document PRAPARE assessments as part of care planning.
