Job summary

Mercy in Missouri is seeking a Nurse Navigator to lead and coordinate patient care across inpatient and outpatient settings. You will assess care needs, guide care plans, educate patients, and ensure timely, safe, and high-quality delivery of services.

The role emphasizes collaboration with bedside nurses, adherence to standards, discharge planning, and performance metrics to optimize LOS and utilization. RN licensure and 2–3 years of experience are preferred.

Qualifications

• Graduate from approved school of nursing.
• Current RN license in state of practice.
• 2-3 years’ experience required.
• BLS (Basic Life Support).
• ACLS Advanced Cardiac Life Support preferred.
• Position requires ability to lift 50 pounds and stand/walk during shifts.

Responsibilities

• Pre-op education for patients.
• Daily rounding while the patient is in the hospital.
• Coordinate discharge planning and follow-up care.
• Uphold Mercy standards for safe, high-quality care.
